MAI Integrated Primary Care and Behavioral Health Services

The MAI-IPCBH model ensures a holistic approach to participant health by integrating behavioral health services with primary medical care. The participants are Broward County residents with HIV/AIDS who are uninsured or under-insured, have barriers to economic stability, and have no other means or funding source to receive services.

Medical Case Management (MCM) & EHE Medical Case Management

MCM provides participant-centered care coordination to improve health outcomes for people with HIV by supporting self-management and preventing complications from co-existing chronic conditions. Services include comprehensive health assessments, development of an Individualized Care Plan (ICP), coordination of medical and support services, adherence counseling, and healthcare monitoring. MCM ensures timely access to care, continuous client monitoring, and advocacy for service utilization. Additionally, providers assist clients in accessing benefits programs such as Medicaid, Medicare, and the AIDS Drug Assistance Program (ADAP).

Non-Medical Case Management (NMCM)

NMCM enhances access to and retention in essential medical and support services by providing coordination, guidance, and assistance with social service needs. NMCM includes assessment, planning, and referrals to programs such as Medicaid, Medicare Part D, and pharmaceutical assistance programs. Services are delivered through various communication methods, including face-to-face and telehealth. The goal is to support participant wellness and autonomy, ensuring retention in care, sustained viral suppression, and adherence to medical treatment plans.

Food Bank, Food Vouchers, EHE Food Bank, EHE Food Vouchers

Food Services offer nutritious food options to support participants’ health needs. These services include Food Bank services, the Central distribution that provides nutritious groceries and personal hygiene products. Food Voucher services are Certificates or gift cards for grocery stores that allow clients to purchase nutritious food and acceptable hygiene products. Lottery tickets, alcohol, and tobacco are prohibited with these vouchers.
